Why won’t he or she fill it out? Do you have a disqualifying illness, or is your doctor just ignorant and/or lazy? If it’s the latter, find another provider to fill out your forms (and take over your care).If it’s the former, they’re trying to help you, so talk to them about what you need to do to qualify for the DMV evaluation.
Can I use my recent I-94 to register for the DMV’s written test?

How do I apply for a driving license in the USA on an F1 visa?
Go to the local DMV with your passport, I94, I20, address proof (utility bills or bank statement within the past 90 days) and your SSN. They will direct you to the officer. You will undergo an eye test (w or w/o specs or lens- as per your vision) then and there. The application and documents will be filled by them referring to the documents at hand. Once you pass the eye test, a minimum fees ($20 in IL) will have to be paid to give the written test. It consists of 35 questions, out of which 28 should be answered correct. Once passed, a permit will be given that can be used to learn driving and later to give the road test.Please note that though the learners’ permit is usually valid for a year, in the case of F1 students, it will be valid for a year or till the end date on your current 120, whichever comes earlier. So, if you are applying towards the end of your coursework, it is most likely that it will be valid only till the end date on your i20.Good luck.

While I was in Phoenix AZ on H-1B in 2007(4 years remaining on my H-1B) - i was given the license for 20 years. My spouse was given for 4 years(she was on H4). But i was charged twice the amount for the license. It was in Deer Valley DMV. When i approached them I was provided a very bizarre reason ‘Sir, we thought you are citizen”. Back in 2005 - Oak brook, IL DMV(Chicago suburb) they didn’t wrote the apartment unit # in our license…when we approached them we were told “we don’t want somebody to know your apartment unit and kill you that’s why the unit is omitted”..I knew even at that time the DMV person(female) had omitted an error and not ready to accept(forget correction). Those kind of mistakes - correction is free but we were somewhat new in USA so simply she took us on ride.The above two examples are real/fact based. In most of the cases DMV person refers some rule book(based on stay validity) and the validity is as per that. For. e.g in you case if your H-1B has got 2 years - you should get the license for 2 years. Is it just an error or overall sentiment is against the H-1B visa - who knows? You should have at least asked them about it? In general these folks are not educated and not paid top $$$ but better to ask them.This time in Carrolton, TX(suburb of Dallas TX) DMV- me and my spouse both were issued 5 years of license(even when i am USC and she is GC). I didn’t had time to even ask why it was done?
